ACCESS TO YOUR HEALTH RECORD

You have the right to access your personal health record.

  • You can make a request by asking verbally, in writing or by telephone.
  • No reason needs to be given.
  • We will contact you about making a suitable time to view the information
  • You can choose to make an appointment with your doctor so they can explain the contents of your record.

Policy

The Practice has developed a Policy and Procedures Manual to comply with AGPAL accreditation expectations.

All aspects of the organisation's activities are documented. Appropriate actions and expectations are covered to ensure all staff have clear guidance about their roles.

The Maual is currently being reviewed as part of our accreditation cycle.

Privacy Information

Your Privacy. Your  medical records are a confidential document. It is the policy of this practice to maintain the security of personal health information at all times and ensure this information is only available to authorised memebers of staff.  Your prior consent will be obtained if any information which may identify you is to be released.

Appointments

You are able to make appointments to see the doctor of your choice.

Normal appointments are booked on a 15 min basis. It is possible to request a double appointment if you believe that you will require extra time with the doctor.

Items like suitability to drive a motor vehicle or Heavy Vehicle Licence will require longer appointments.

Please discuss your needs with our reception staff when booking appointments.
