Policy

  • Appointments are usually for one problem only - If you would like to discuss two items please ask for a longer appointment. Please phone 6299 6990
  • Scripts - It is safer to make an appointment for repeat prescriptions.
  • Travel Vaccinations- Please ask reception for a Travel Questionnaire. Please fill this in before you see the doctor.
  • Referrals- The doctors encourage you to make an appointment for referrals. They last 12 months. If you need ongoing specialist care referrals can be updated every 12 months by asking your doctor (at an appointment) to provide an updated annual Care Plan.
  • Sickness Certificates- These can not be backdated but the doctor may be able to issue you with a  Letter of Support for a previous day off work provided that you are still sick on the day that you visit. Extensions over the phone are not permitted for legal reasons.
  •  Workers Compensation- all visits must be paid for by the patient on the day of the consultation.
  • Annual Care Plans- We aim to provide all patients who have long term health issues with a comprehensive written Care Plan.  Our Nurse can assist you with this.
  • Medical Examinations- It may be helpful to bring a specimen of urine with you.  A small yellow jar is available from reception for this purpose. Please make sure that you have filled in all the details on any questionairre that you might bring with you.
  • Patient Feedback - We encourage positive and negative patient feedback, on any area within the practice. If you have feedback, you can provide this anonymously at the practice using our suggestion box. All feedback is considered. Concerns may be raised with the Practice Manager by sending a letter addressed to the Practice Manager. Any concerns will be considered by the Practice Manager and feedback given to you promptly. We would appreciate the opportunity to deal with any concerns within the practice. If you do not feel your complaint can be addressed at a practice level, you may direct it to the NSW Healthcare Complaints Commission on 02 9219 7444.  

 

Privacy Information

Confidentiality is extremely important at Brindabella Family Practice. All information in your file is confidential and will only be released to a third party with your written authority or by court order. All staff sign formal confidentiality agreements. We ensure all patient consultations and medical records remain strictly confidential.
