Policy

Grace family Practice is a non smoking area
No food is allowed inside the practice
Appointments are to be kept promptly. A fee is incured if patients don't turn up without cancellation more than 2 times.
Patient need to consult the doctor to get ant repeat prescriptions. A fee of $10 is incurred for any prescription given over the phone, if the patient hasnot consulted the doctor in past 2 weeks.
Patient has to consult the doctor for any referral letters to the Specialists. No referral letters are given over the phone.
The blood or X Ray results will be informed to the patients only by the doctor, nurse or a relevant medically qualified person (eg: diabetic educator, dietetian etc.). No reception staff or medically unqualified person is authorised to access or give results to the patients. The results will not be revealed to a person other than the patient (except children <16yrs.) without the consent of the patient. If doctor has any concerns regarding your results our staff will contact you to make an appointment to discuss the results
Because of the uncertainity of emails reaching the specified patient, the Practice discourages the use of email consultation for medical advice.

Privacy Information

All information about you held in this practice is kept in confidence because we value your privacy.
All patients' records are kept confidential.
With the introduction of the Privacy Act Amendment (2000) we remain committed to protecting your privacy.
The information from the patients will not be disclosed to any other person without the consent of the patient except in circumstances where the information could cause danger to the patient or others.
Patients will be asked to sign a consent form to allow those involved in your healthcare, access to the information necessary to continue the high standard of health service patients expect of us. (This includes Pathology, Radiology, Specialists, Allied Health etc.)
