Practice Policy Statement

To provide a courteous, appropriate, accessible and efficient service of support, treatment and advice to all patients and those involved in their care. To maintain effective cooperation with ancillary services in order to involve them as appropriate in the patient's interest. To maintain records that accurately shows the significant medical history, medications, reports and all medical contacts with each patient; To provide information, advice, and counselling on health maintenance and health improvement.

To ensure and maintain complete Medical Confidentiality, no Medical record or any other information obtained in the Practice concerning a patient is ever to be disclosed to any other person(s) than the patient except for a necessary medical reason of benefit to the patient, or legal reason on production of appropriate authorisation. Medical confidentiality being paramount at all times, and any other use of the information in the records is subject to the informed consent of the patient involved.

A Practice Policy and Procedures Manual is maintained and amended as required to update with any changes in circumstances. Each member of the staff is to be aware of the contents of the Manual and to be provided with a copy on request.

There is an undertaking to maintain Practice Accreditation standards at all times and for all the doctors to remain Vocationally Registered.

Patients are entitled to make their own decisions about medical matters and procedures and should be given adequate and unbiased information on which to base their decisions. This includes a choice to opt out from any treatment, or research program, at any time, Informed consent must always be obtained before providing any data, which could lead to tile identification of the patient.

 

The practice operates a No Smoking policy.

HOME VISITS Home visits are restricted to patients of the practice.

AFTER HOURS The practice is associated with National Home Doctor Services which offer after hour care for patients of this surgery.

Limited Privacy Policy 

Family Practice has developed this policy to protect your privacy in compliance with privacy legislation.

 

Our policy is to inform you:

1. What personal information is being collected;

2. Who is collecting your personal information;

3. How your personal information is being used; and

4. To whom your personal information is being disclosed.

 

Collection, Use and Disclosure of Personal Information by Family Practice: We do not collect any personal information on this website.

Appointments

Please ring 02 9977 6543 for an appointment.

Every effort will be made to accommodate your preferred time.

Emergencies will always be given priority and our reception staff will attempt to contact you if there is any unforeseen delay or your doctor has been called away.

 

Longer consultations times are available so please ask our receptionists if you require some extra time. If you or a family member requires an interpreter service we can organise this for you, please let us know wher you make the appointment.
