Your Privacy: Our Policy

The provision of quality health care requires a doctor-patient relationship of trust and confidentiality.  Consistent with our commitment to quality care this practice has developed a policy to protect patient privacy in compliance with privacy legislation.

  • That we need your consent to collect information about you
  • Why we need to collect your information
  • How your information will be used by us and to whom we may need to disclose it
  • That you may request access to the information we hold about you.
  • That you may discuss any concerns you have about how we handle your information.

Personal Health Information

Your details, medical records and consultations within this practice are kept strictly confidential.  Your personal health information is only accessed by authorised staff and will not be released without your written consent.  Only de-identified data may be used for research purposes.  During the accreditation process medical records are randomly selected by a GP for quality assurance purposes.  If you do not wish for your record to be accessed please inform your doctor.

Appointments

All appointments are by bookings only.

 

In the case of new referrals for Prof Wittert all referrals are to be faxed to the practice first and Accessed by Prof Wittert in reference to urgency and the patient is then rung with appointment time.

 

Dr Koopowitz has currently not taking new patients.
